Full Job Description

You will improve clinical outcomes for patients by enabling them to function independently by increasing their choice to remain in their own home/community and reduce the need for, or prevent inappropriate hospital admission., We aim to recruit clinicians who are passionate in supporting and empowering patients and their family/carers to manage their complex health needs.
We strive to support patients accessing help when needed and optimising health outcomes.
We want to support patients to remain in their own homes as appropriate preventing unnecessary admission to hospital and support patients to return home as needed.
Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.
We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people's own homes and via virtual consultations.

We deliver a variety of services across Derbyshire, caring for nearly 1.8 million patients each year.
