Full Job Description

As a Senior Community Health and Social Care Support Worker, you will deliver high quality care to people across Derbyshire, promoting independence, dignity and wellbeing. Working alongside registered practitioners, you will provide clinical, therapeutic and non-clinical interventions within your scope of competence, following organisational policies and clinical standards to ensure safe, compassionate and effective care.

Supporting people in their own homes or within the Community Support Bedded Unit, you will work as part of an integrated multidisciplinary team to deliver coordinated care in the most appropriate setting.

In addition to providing direct care, you will take on day-to-day operational leadership during your shift. Acting as the first point of contact for Community Health and Social Care Support Workers, you will provide guidance, support and direction to ensure the smooth running of the service. You will work closely with registered practitioners, Service Managers and other healthcare professionals to coordinate care and maintain effective communication across the team.

The role also includes responsibility for the safe administration of medication in accordance with organisational policies, procedures and training.

We're looking for caring, dedicated Senior Support Workers to join ourCommunity Support Bedded Unit in the High Peak.

Our unit is located in the High Peak, where public transport links are limited, so applicants should be able to travel to and from the service reliably.

If you're passionate about making a real difference and enjoy working as part of a supportive team, we'd love to hear from you.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people's own homes and via virtual consultations.
