Full Job Description
Join one of our self-managing teams. The teams have professional autonomy and responsibility for their own day to day running; working to support and empower each other to deliver the highest standard of person-centred care. The teams are self-managing where designated responsibilities for the team are shared and specific roles within the team are rotated i.e. allocation of clinics, team rota construction. Therefore, your role within the team is to support the ethos of a self-directed team with the authority to make decisions about workload, recruitment, training requirements etc We can offer flexible working hours. The usual hours are 7am to 6pm, 3 days a week with 1 half day from 7am to 1pm or 12pm to 6pm. The service runs 7 days a week so you will be expected to work at weekends occasionally (approximately one in every 8 weekends). To work as a member of a self-directed wound clinic team that delivers person centred integrated care in a clinic setting. Care will be focussed on improving self-reliance and empowerment; seeing patients as a whole and supporting as required their health, social and psychological needs. To work as an integral part of a clinic-based team providing consistent care to a defined clinic caseload of patients 365 days a year. The team sees mobile patients with a wide range of wounds including diabetic foot ulcers, leg ulcers, PUs, post-surgical wounds, burns and supports patients with a range of treatments including suture removal, topical negative pressure, compression therapy, etc. The role requires close collaboration with the GP and other health, social care and voluntary organisations including but not limited to therapists, mental health, social workers, care navigators, case managers etc. Your role will support the team to achieve the following core aims: 1. Having happy patients: through direct access, continuity of care, flexibility of care provision and providing quality clinical care. 1. Having happy nurses and care givers; by working collaboratively and flexibly with your team colleagues and being empowered to make decisions as a team to best meet the requirements of your patients and their families. 1. Self-empowered patients who will work jointly with the team to achieve agreed outcomes.
