Full Job Description
This post is available for RMHN's, Occupational Therapists and Social Workers. The post holder will undertake screening , triage and in some cases full assessment of new referrals to the PCMHT. This will include urgent assessments of 17 year old or rising 18 year olds at crisis point and short and medium term care co-ordination. The post holder will offer urgent assessment slots as part of the urgent care pathway. The post holder will be a care coordinator, assessing and treating and advising and signposting a specific group of young people in the team working in collaboration with the multiple disciplinary teams (MDT). The post holder will contribute to the shaping of this service together with various clinical services and young people and their families.Main duties of the job Participate as a multi-disciplinary team member in the assessment of children and young people referred via CAMHS, primary care and Slam services Ensure that all recording and reporting of client contact are accurate, up to date, of good quality and within PCMHT and professional requirement standards of record keeping. To collect & collate information about individual clinical activity on ePJS Work with the team and clients to maintain a safe therapeutic environment. Work to assure the quality of care through achieving high operational standards in clinical systems To maintain and ensure good professional standards and attitudes towards the care and treatment of children, young people and their families To link with other statutory and voluntary agencies and professionals as well as other CAMHS Teams in terms of referrals and joint work. Maintain good relationships and clear lines of communication with other professional agencies and departments, providing specialist advice and support where appropriate Contribute to the effective functioning of the team and service through attendance at relevant meetings, taking responsibilities within the service as appropriate and negotiated with the line manager. To have responsibility to ensure you achieve positive clinical outcomes for young people as they transition through the services and that these outcomes are evidenced by performance indicators such as activity data, targets and quality measures.
