Full Job Description
We are looking to recruit an experienced band 4 Senior Support Worker as a member of our established Adolescent Outreach Team. The team are really fortunate that we have an experienced full multi disciplinary representation, and high levels of retention of staff. The MDT are skilled and highly committed to supporting the wider network, parents and carers to support young people to remain at home, and if required to come to hospital facilitate a safe discharge and reduce length of stay. The post-holder will work within AOT as a Senior Support Worker facilitating individual therapeutic care plans/ therapeutic interventions that have been planned with or by qualified members of the team. The post-holder must be committed to effective team working and to the coherent delivery of the care this will include individual, family, group work and telephone support. The team has strong working relationships with our local inpatient service (Aquarius), SLP inpatient units (Snowsfields and BAU), Tier 3 community CAMHS, CAMHS Emergency Care Team (CECS), CAMHS Crisis Line and CAMHS Bed Management. This is a fantastic and exciting opportunity has arisen for a new Senior Support Worker in AOT
- The post holder will facilitate individual therapeutic care plans/ therapeutic interventions that have been planned with or by qualified members of the team. This may include supporting and escorting young people to appointments with key agencies, assisting with behavioural programmes, advocating for the young person and co-facilitating therapeutic groups or activities or providing assistance in a crisis.
- The post holder will also work very closely with families/carers to help support and provide education in how to best manage a young person's serious mental health problem. This may either be in a group setting or within the home.
- This role involves independent daily travel across South West London and involves some lone working, within a close supervision structure., Our largest, 19-ward site is in Springfield Village, created following our £150m investment into two new world class mental health facilities. A 32-acre public park, shops and cafes are coming soon. Close to shops, cafes and Tooting Bec on the Northern Line plus Earlsfield and Tooting rail stations and bus routes.
- To carry out individual programmes of care with identified young people who require extra input for a period of time e.g. around attending the Aquarius Unit day programme, acquiring confidence in using public transport or engaging in other community-based activities. This will also involve supporting discharge programmes for inpatient young people by assisting them in employment, education, social and leisure activities. Care will be planned by or with qualified members of the AAOT.
- To be able to work in a supportive and educational way with families/carers and be able to portray the confidence and competence to manage the challenges that this can portray within the home setting.
- To perform delegated duties without direct supervision from qualified staff, within the home or community, e.g. supporting young people in attending appointments with other key agencies, supporting young people in gaining confidence in public settings and facilitating individual behavioural programmes.
- To co-facilitate therapeutic groups with other members of the team.
- To be flexible within the given working hours and to be able to work over the service allocated time period of 08:00am and 20:00pm (weekdays) & 11:00am- 19:00pm (weekends & bank holidays). The shift period will vary throughout the week from 08:00am to 16:00hrs and 12:00 to 20:00hrs.
- To be proactive in gathering information on and creating positive working relationships with local services and activities for young people within the 5 boroughs covered by the Trust, as well as creating and compiling resources within the service.
- To support and assist young people with Housing or Benefits applications or enquiries.
- To provide support to families and carers, including assisting in the running of parent and carer support groups where necessary.
- To attend and contribute to clinical team meetings either within the hospital or in Out-Patient CAMHS. and to initial assessments of new clients referred into the Service.
- To maintain accurate records in the case notes in accordance with the CAMHS Standards for Clinical Records.
- To participate in the orientation and induction of all new members of staff.
- To be aware of and to implement both the Trust's and the Adolescent Resource Centre's Policies and Procedures.
- To maintain a professional working relationship with all staff, parents and carers, young people and other agencies.
- To maintain professional standards in accordance with the Trust's Code of Conduct for Health Care Assistants.
