Full Job Description
The post holder will assist in the delivery of care plans and programmes by - implementing programmes and reporting outcomes, contributing to the recording of plans/programmes and providing therapeutic activity where agreed and appropriate.
Acting to as a nominated worker for individual clients who have learning disabilities and may also have extreme challenging behaviour including physical and verbal aggression, changes in mental health, forensic needs, complex physical health needs. Facilitate and support the development of appropriate personal care and independence with activities of daily living. Support self-help, communication and social skills acquisition
The service model is one of supporting people, whose behaviours challenge services, working closely with specialist practitioners and other health professionals to support this complex group of individuals to remain in their community setting and, where an appropriate admission has occurred, to be discharged promptly and safely in line with national Transforming Care strategy.
Main duties of the job
To promote and champion the rights of people with learning disabilities, working with the current service philosophy and values as detailed in Transforming Care for people with a learning disability
Support the delivery of specialist health support which promotes the independence, community inclusion and positive reputation of people with learning disabilities.
To assist the Enhanced intensive support part of the LD community team with care plans and programmes by :
Implementing programmes and reporting outcomes.
Contributing to the recording of plans and programmes.
Act as a nominated worker for individual clients who have learning disabilities and may also have extreme challenging behaviour including physical and verbal aggression, changes in mental health, forensic needs, complex physical health needs
Facilitate & support the development of appropriate personal care
Facilitate & support self help, communication and social skills acquisition
Assist and promote independence with activities of daily living
Facilitate recovery by assisting participation in social, vocational, recreational and leisure activities
Participate in the implementation of specific programmes of care or therapy to develop and enhance socially appropriate adaptive behaviours under the supervision of qualified staff
Demonstrate a range of communication skills & techniques to enable positive communication to take place with individuals who may have complex communication needs.
