Full Job Description
This role will also support the medicines management aspects of this initiative through working within the general practice setting with the support of a Band 8 Clinical Pharmacist. This role will work as part of the general practice team to provide specialist advice to patients, and undertake full patient cantered medication reviews to proactively manage people with complex polypharmacy, particularly the elderly and those with multiple conditions., Serving approximately 68,500 patients, North Lambeth Primary Care Network (PCN) is working to support and strengthen the delivery of primary care services in each of its 6 member practices, by working collaboratively and delivering quality improvement initiatives. We aim to
- achieve excellence in patient care and outcomes;
- maximise practice income achieved under contracts, including PMS KPI, QOF, GPDF and other local enhanced services;
- release clinical capacity by improving efficiency and effectiveness;
- enhance our workforce by providing training and providing interventions and support that develop resilience.
We are looking 1x enthusiastic, highly motivated Clinical Pharmacist to join our team and contribute to developing our services. Applicants for this specialist pharmacist role must be suitably qualified and will preferably have experience in an acute hospital setting or a GP Practice. The successful candidate will be required to work at any of the Practices within the North Lambeth Primary Care Network, Candidates should be a registered pharmacist with post registration experience, have well developed communication skills and a patient focused approach to problem solving.
