The successful applicant will be an NHS employee and may therefore have contact with vulnerable service users. Unless exempt, we encourage and support staff to be vaccinated as this remains the best line of defence against COVID-19., + The post holder will interrogate a number of health and social care systems to gather relevant information aimed at enabling qualified staff to appropriately triage an assessment visit.
+ · The post holder will feedback to the referrer as well as providing relevant information to the integrated team.
+ · The post holder will register and discharge patients to the relevant CFT system.
+ · The post holder will be expected to maintain the highest levels of confidentiality and carry out relevant administrative duties as required.

We're an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people's physical and mental health. We also provide specialist support to people with dementia or a learning disability.

Approximately 568,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon. In the summer, and during other holidays lots of people choose to visit the area. This increase the numbers of people who use out services.
We are seeking an enthusiastic and experienced administrator to join us whose primary function will be to manage referrals via a variety of media from GPs, hospitals, specialist teams and Adult Social Services via various referral systems for the Penwith Integrated Community Team as well interacting with patients, carers and other health and social care professionals.
The integrated team comprises Penwith Community Nurse Team, Penwith Community Rehabilitation Team, Penwith Home First Team and Penwith Community Matron Team. The integrated team covers the local area including Hayle, Marazion, St Ives and Penzance and surrounding areas.
The successful applicant will be articulate and have a keen eye for detail. Ability to maintain patient and staff confidentiality is an essential aspect of this role.
If you have excellent communication and organisational skills and would enjoy working within a busy team we look forward to hearing from you. Experience of working administratively within healthcare/NHS preferred.
