Full Job Description
As a vital part of our multi-disciplinary team, you will contribute to delivering a patient-centred rehabilitation service, working closely with occupational therapists, physiotherapists, and other healthcare professionals.
If you are looking for a rewarding role where you can actively contribute to patient recovery and rehabilitation, we'd love to hear from you., Deliver rehabilitation through therapy interventions, which may include support with personal care tasks, cognition and mobility
Carry out rehabilitation plans as delegated by occupational therapists and physiotherapists.
Maintain accurate documentation and communicate effectively with service users, families, and the therapy team.
Be flexible to support the team during peak service times.
Participate in the 7-day service rota, including weekends, delivering rehabilitation between 8am and 8pm.
Passionate about patient care and rehabilitation.
Excellent communication and teamwork skills.
Ability to work independently and as part of a multi-disciplinary team.
Organised with good time management skills.
Experience in a healthcare or therapy setting is desirable but not essential., This is a community post and therefore the successful candidate will require to the ability to travel between sites in a timely manner.

Please be advised that there is a temporary top up for Bands 2 and 3 to reflect the incorporation of the top up to the living wage of £12.60 per hour - £24,638 per annum.
This temporary top up will be in place until the annual pay uplift for 2025/26 is confirmed.
