Full Job Description

This is an exciting pioneering role for carers seeking opportunities for professional development in Health & Social Care.

We are looking for part-time Band 3 Community Support Workers to support holistic and personal care for people at the end of their lives, ensuring the best possible outcomes for people in our care.

This role involves providing domiciliary care for a wide variety of patients, offering a short-term service whilst a longer-term package of care is being secured.

Patients will be supported in their home environment to prevent avoidable hospital admissions, facilitate earlier hospital discharges as well as in-reaching to the wards.

We are committed to providing excellent standards of care to our patients and we know that to enable us to do this, the continuous professional development of staff in a supportive environment has to be a top priority.

In exchange for your enthusiasm and commitment, you will be provided with a comprehensive introduction into care with training and ongoing support to give you the skills you'll need to be flexible in your approach and confident to work alone.

Applications from candidates that require current Skilled worker sponsorship to work in the UK are unfortunately ineligible to apply for this position.

The main duties are to work with District and Community nurses, who will lead the team and delegate wellbeing care plans, to provide support for patients having palliative care at home. this could include personal care, help to move, pressure area care, minor wound care, prompting medication, help with eating and drinking, ensuring a clean and safe environment, promoting independence and health and wellbeing.

Emotional support and mental health is also considered; signposting to appropriate services for palliative and end of life care. The team work proactively and independently as well as reporting back and communicating effectively with the multidisciplinary team. The team also participates in assessment of people's needs, contribute to preventing avoidable admissions and supporting earlier discharges from Acute and Community Hospitals for End of Life Patients.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
