More information about the role We have an exciting opportunity for an enthusiastic and motivated Band 3 Rehabilitation Support Worker to join our Community Therapy Team within the HOSM Community Rehab Team based at Honiton. The Community Rehabilitation Team comprises Occupational Therapists, Physiotherapists, Rehabilitation Nurses and Band 3 Support Workers who work collaboratively and in partnership with our UCR team to support the rehabilitation of patients in their own home as well as care homes to achieve patient goals. The ideal candidate will have an interest in community rehabilitation, multi-disciplinary working, falls prevention and management of patients with a wide variety of short and long-term conditions and carry out treatment plans within your competencies or as delegated by registered staff. Working Pattern: 37.5 hours per week. Our standard working pattern is 0830-1630 Monday to Friday but you will also be expected to work alongside Urgent Community Response and support
weekend and bank holiday working on a rota basis.
