Full Job Description
Have an Enhanced level of knowledge in the prevention and management of the deteriorating patient utilising the NORF Enhanced Practitioner Framework.Appendix-1-Enhanced-Critical-Care-Outreach-Competencies.pdf
This autonomous role will exercise independent judgement to assess, investigate, plan, implement, evaluate and escalate clinical care of the deteriorating patient.
Provide patient centred care to a high standard usually within complex and evolving clinical scenarios utilising their enhanced clinical knowledge and expertise.
Make complex clinical decisions and work collaboratively with the multidisciplinary team to support the care of patients including discussions surrounding treatment and escalation plans and DNACPR discussions.
Support clinical teams across the organisation including Medicine, Surgery, Maternity, Emergency Medicine and Paediatrics by providing enhanced clinical advice, demonstrating leadership and support to ensure effective and safe provision of care to the "deteriorating patient".
Support, deliver and evaluate education, research and audit activities to elevate and improve standards associated with the ACRT service.

If you do not hear back within 3 weeks of your application, please assume you have been unsuccessful on this occasion., You will be required to take responsibility for triaging, coordinating and overseeing the organisation of patient care for acutely ill ward-based patients on a day-to-day basis.
You will work within own scope of practice and in collaboration with the multi-professional team to instigate, evaluate and modify a range of diagnostic testing /interventions within the Acute Care Response Team. You will also directly refer to relevant Clinical specialities where appropriate.
You will work closely with the Critical Care Team to facilitate early and timely admission to critical care and safe discharge from critical care.
You will be required to respond to medical emergencies and demonstrate clinical leadership and expertise as required.
You will support the Deteriorating patient team in developing and delivering robust teaching and education programs to ward teams to manage acutely ill/deteriorating patients, and support the development and integration of Martha's Rule across the organisation.
The role as ACRT Enhanced Practitioner is underpinned by the Critical Care Outreach Practitioner Framework. and supports a 24/7 service across the Watford site and currently Mon-Fri Night at St Albans which aims to prevent, identify, escalate and respond to patient deterioration along with supporting successful transition of patient care from the critical care environment. The Enhanced Practitioner (Band 6/7 depending on experience) will be able to assess and treat patients in a variety of clinical scenarios.
The post holder will demonstrate enhanced skills and competence in areas such as advanced physical assessment and ensure that evidence-based practice underpins all aspects of care and treatment. They will have developed enhanced skills in critical thinking and problem-solving to support safe clinical decision making. They will deliver a high level of specialist care to patients outside of the Critical Care Department working within a defined scope of practice that is underpinned by both local and nationally agreed guidelines.
