Full Job Description
As part of the Medicines Management Team, you'll be responsible for ensuring accurate medicines reconciliation, supporting safe and timely discharges, maintaining high standards of medicines optimisation across designated wards. You will play a key role in managing patients' own drugs, conducting controlled drug reconciliations, and helping to maintain efficient ward stock systems., + Complete a 'medicines reconciliation' for all new admissions according to national standards, which will include accessing the Trust Electronic Health Record, contacting hospitals, GP surgeries, community pharmacies and talking to service users/carers to compile a complete and accurate list of current medication + Conduct Controlled Drug reconciliation on a three-monthly basis and report results to Site Lead and ward/department managers + Be responsible for the maintenance of the Patients Own Drugs (PODs) scheme for designated wards according to Trust policy + Train nursing, medical and pharmacy staff in the use of PODs + To supply non-stock items according to department procedures + Dispense short leave and discharge prescriptions on site, where appropriate + Work with clinical teams to review and update ward stock lists for designated wards Please refer to the job description attached for a comprehensive list of duties, We hope that the advert has given you a clear understanding of the skills we are seeking and the opportunity at hand. We are seeking a GPhC registered Pharmacy Technician with an NVQ Level 3 in Pharmacy Services or equivalent, and experience in a pharmacy setting. Strong communication and organisational skills are essential, and experience in a hospital or secondary care environment is desirable. Medicines optimisation is at the heart of the service we provide in Oxfordshire's Community Hospitals, so if you are passionate about delivering high-quality care and want to make a meaningful impact, we would love to hear from you! The Oxford Health Pharmacy & Medicines Management service employs approximately 80 staff, predominantly clinical pharmacists, and technicians, supported by dispensary and administrative staff. Clinical pharmacists and medicines management technicians (MMTs) are working at clinical sites with supply and support functions at hubs in Oxfordshire. The department provides services to our community hospital and mental health patients, alongside other community-based patients and teams.
