Full Job Description

We are looking to appoint a committed, highly motivated, enthusiastic, and skilled Band 5 Staff Nurse to join our dynamic, integrated multi- disciplinary Rapid Response Service (RRT). RRT was one of the 7 accelerator sites involved in the introduction and successful roll out, of the NHSE's Urgent Community Response initiative. RRT is a multi- professional team consisting of Nurses, Physiotherapists, Occupational Therapists and Social Workers. The team aims to respond to crisis referrals within 2 hours and avoid unnecessary presentation to hospital and residential care, by supporting community patients who are in crisis and prevent direct hospital admissions. The team provide holistic health and social care assessments for people over the age 18 years, who are registered with a Bexley GP and live in the Borough of Bexley. The team are based at Goldie Leigh Hospital Abbey Wood, SE2 0AS - 7 days a week, on an 8am-8pm basis, providing an urgent community response to vulnerable people in the community. Currently based at QEH but outreach to other acute hospital/community sites respond to urgent referrals from A&E, UTC, LAS 111, Acute Medical Unit, Short Stay Unit, Clinical Decision Unit, Day Care Unit GP's and other community partners. The post holder will work as a part of the multi-disciplinary UCR - Rapid Response Team including Consultant Geriatrician, Senior Nurses, Occupational Therapists and Social Services colleagues to meet the needs of patients referred to the team, ensuring the highest possible standards of practice with a strong emphasis on falls, frailty, breast drain care and outpatient parenteral antimicrobial therapy,OPAT allows patients requiring intravenous antibiotics to be treated outside hospital. The post holder will conduct comprehensive assessment nursing assessments for patients within acute and community settings. They will need to demonstrate sound clinical reasoning and risk assessment skills to guide appropriate nursing intervention and when appropriate avoid unnecessary A and E presentation. The post holder will also have the opportunity to work closely with our Community Consultant Geriatrician. The team work directly with (LAS) and act as a liaison between Emergency Departments, Community and Care home settings, by identifying the most appropriate onward referrals and initiating integrated care pathways, ensuring seamless provision of care and the instigation of on- going case management of long term condition management. The Rapid Response Team provides community follow up and support and review for up to 5 days. Post holder will undertake delegated line management of junior staff, both informal and formal basis., To be responsible for receiving urgent referrals from acute and community settings, including London / Kent Ambulance Service, locality teams, community team, primary care services, local voluntary organisations and take appropriate action. Partnership working with acute setting in assisting with 4hour breech times. Identification of specific and appropriate patient care pathways, to maximise holistic patient care and prevent unnecessary admission to acute setting. Breast drain care, Wound Care, cannulation, IV Therapy Daily liaison between acute setting and community services aiming to facilitate seamless multi-disciplinary care provision. Undertaking timely and appropriate referrals to existing community services to ensure ongoing care intervention and effective case management. Undertake when appropriate urgent community visits/follow up visits after discussion with senior practitioner(s). Direct communication and joint working with GPs to ensure transparency of information, prevention of unnecessary admission and ongoing patient community case management. Responsible for keeping up to date with current issues and events in Primary HealthCare. As appropriate participate in nursing developments and project work and take an active role in new developments within Primary Care. To participate in multidisciplinary activities such as integrated team meetings To liaise with other agencies and refer patients, their families and carers as appropriate. To actively participate in the development of the service working collaboratively with other care providers. To work flexibly and relieve colleagues/rotate within the team as required and requested by the team leader to meet the needs of the service.
