Full Job Description
The Senior Pharmacist will undertake clinical medication reviews and patient facing clinics, working in partnership with the patient's GP practice, nurses, care homes, community pharmacists, secondary care and the local authority. Utilising their prescribing qualification, the role will support the medication needs of the patients as well as educate other healthcare professionals on prescribing and the effective use of medicines.,
- Medicines optimisation working closely with practice management to deliver key prescribing objectives in structured medication review
- Care home clinical services
- Antimicrobial stewardship
- Personalised pharmaceutical care, transfer of care
- Integrated care with community services
- Prescribing safety and governance.
- Audits e.g. High risk drug monitoring
The successful applicant will need to be self-motivated and able to work on their own, whilst also integrating into a well-established team. You will have excellent organisational, interpersonal and communication skills together with a good working knowledge of primary care and the challenges facing medicines management in the new era.
