Full Job Description
Are you a dynamic and experienced leader ready to make a real impact in adult social care? We are seeking a Service Manager to lead our Transfer of Care Team and Continuing Healthcare (CHC) services. This is a pivotal role focused on ensuring safe, timely, and person-centred hospital discharges and effective CHC coordination across our system. You will be responsible for the operational management of hospital discharge pathways, supporting five Team Managers and working closely with the Head of Service to meet performance targets and improve outcomes for residents. You'll also oversee the interface with Continuing Healthcare, ensuring robust processes and collaborative working with health partners. This is a fast-paced, high-pressure environment that requires a confident, resilient leader with a strong grasp of adult social care legislation, discharge planning, and integrated working.
- A qualified Social Worker registered with Social Work England (or equivalent health qualification).
- Proven experience in hospital discharge, Continuing Healthcare, or integrated health and social care.
- Strong leadership and operational management skills, with the ability to motivate and influence others.
- Excellent communication, negotiation, and partnership-building abilities.
- Comfortable working with data, managing budgets, and leading change in a complex system.

- Lead and manage the delivery of high-quality, efficient discharge and CHC services.
- Support and develop Team Managers and frontline staff across multiple sites.
- Drive performance improvement and ensure compliance with statutory duties.
- Collaborate with NHS and third-sector partners to ensure seamless care transitions.
- Manage risk, safeguarding, and service quality with a focus on continuous improvement.
