Assess patients' mental, physical, and social care needs and contribute to comprehensive care planning.
Implement and regularly review individualised care plans in collaboration with patients and multidisciplinary teams.
Administer medication safely and monitor patients for therapeutic effects and potential adverse reactions.
Provide emotional support, therapeutic interventions, and appropriate guidance to patients and their families.
Monitor patients' mental health presentation, behaviour, and overall wellbeing, escalating concerns where appropriate.
