conduct comprehensive assessments of patients health needs
develop implement and review individualised care plans
provide appropriate treatments and interventions as required
undertake treatment room responsibilities and deliver chronic disease management clinics
carry out specific clinical tasks including but not limited to
work collaboratively with gps other nurses and the wider healthcare team to ensure high quality patient care
maintain accurate timely and confidential patient records in line with nhs standards
deliver health education advice and counselling to patients and their families
keep up to date with current clinical guidelines medical advancements and best practice in primary care

We are looking for a qualified General Practice Nurse, and nurses with relevant experience who are keen to develop their skills in primary care are encouraged to apply.
