We are looking for a reliable, caring, and proactive Support Worker to provide consistent supervision and support Toni in her daily life. This is a varied and rewarding role, supporting someone to stay active, engaged, and connected to their community.,

Providing consistent supervision and support throughout the day
Supporting attendance at regular activities such as swimming, the gym, and crafts
Encouraging social engagement, including meals out and visiting family
Accompanying on trips out, such as seaside visits, garden centres, and nature walks
Attending and supporting with appointments (e.g. GP, hospital, hearing/eyesight tests, Job Centre/DWP)
Patient, reliable, and attentive
Enjoys being out and about and supporting others to engage in activities
Able to provide consistent supervision and build a trusting relationship
Flexible and proactive in supporting day-to-day needs
