We are looking for experienced Community Health & Social Care Support Workers (Band 3) to join our bank and support services across Derbyshire. You will work within small community support bed and assessment units, providing high-quality, person-centred care to adults with a range of physical, mental health and social care needs. This is a bank / ad-hoc role offering flexible shifts rather than guaranteed full-time hours.,

Provide compassionate, person-centred care in line with individual care plans.
Support with personal care, mobility, eating and drinking, and activities of daily living.
Complete basic observations (e.g. blood pressure, blood glucose, urinalysis) and escalate concerns appropriately.
Promote independence, dignity and wellbeing, including social and emotional support.
Work as part of a multi-disciplinary team, following agreed policies, protocols and safeguarding procedures.
Maintain accurate records and contribute to safe, effective service delivery.

Minimum 1 year of UK experience as a Healthcare Assistant / Support Worker in a health or social care setting.
Experience working with adults with physical disabilities, learning disabilities, mental health needs and/or older adults.
Ability to travel reliably to at least one of the three sites (Buxton, Ilkeston, Staveley) for early, late and night shifts.
Good understanding of safeguarding, confidentiality and professional boundaries.
GCSE Maths and English (or equivalent) and one of:
NVQ Level 3 in Health & Social Care, or
Equivalent health/social care qualification, or
Equivalent proven experience in a CQC-regulated organisation with Care Certificate evidence.
Right to work in the UK (we cannot provide sponsorship for this role).
DBS (preferably on the Update Service).
Able to provide references covering the last 3 years.
Willing/able to provide immunization evidence as required (e.g. TB, Hep B, Varicella, MMR).
