Interested? … The adult access team are seeking a social worker to support with the Hospital at Home initiative is an innovative, preventative model of care that supports adults to remain safely at home with the right care and support in place. Delivered in partnership with health services, this model helps reduce unnecessary hospital admissions, improving outcomes and promoting independence. The Access Team plays a vital role in this model by coordinating rapid assessments, care planning, and integrated support delivery.
