Full Job Description
You will be joining a well-established MDT which offers community stroke and neuro rehab pathways. We provide services in the London Borough of Newham in client's homes or appropriate community settings.

The Community Neurology Service provides rehabilitation and management for people who have a neurological condition including stroke in their own home or appropriate community location. The service provides specialist, coordinated advice and treatment to maximise independence, limit disease progression, reduce hospital admissions and facilitate smooth transition home following a hospital discharge.
The role of the Highly Specialist Physiotherapist is to provide a highly specialised client centred neurological physiotherapy service to clients who are referred to the Community Neuro Service.
Main duties of the job
§ To provide a highly specialised client centred neurological physiotherapy service to clients who are referred to the Community Neuro Service.
§ To work as an autonomous practitioner in a community setting managing a complex clinical caseload by applying specialist knowledge and skills to the assessment of the needs of these clients. From the assessment, to formulate a treatment plan designed to reduce the impact of disability and ill health.
§ To manage a defined caseload independently, evidencing problem solving and clinical reasoning skills in line with evidence based/ client centred principles.
§ To be an influential member of the multi-disciplinary team, providing expert, integrated, client-centred, goal orientated rehabilitation for clients with neurological disability. Acting as a key worker for specific clients.
§ To provide supervision, teaching and appraisal for lower banded physiotherapists, assistants, students and other members of the multidisciplinary team.
§ To contribute to the planning, development and evaluation of physiotherapy within the service and to the Community Neurology Service as a whole.
§ To work in support of and deputise for the clinical lead in Community Neuro Service., + To use specialist skills in providing physiotherapy assessments and interventions in various settings (e.g. the client's home, workplace, community).
+ To develop and implement specialist interventions based on evidence and relevant clinical guidelines.
+ To write specialist physiotherapy reports reflecting specialist stroke and neurological knowledge.
+ To provide advice to others regarding the management and care of adults with mobility problems, arising from stroke or neurological conditions.
+ To ensure that patients, families and carers are involved in the planning and implementation of care plans, home exercise programmes.
To demonstrate negotiation skills in the management of conflict across a range of situations
