As a Community Wellbeing Worker, you will play a role in delivering Family Hub and active services across universal provision. Your responsibilities will include:

Linking individuals to professional and community-based support
Planning, coordinating and delivering targeted and bespoke early intervention support for children, young people and families. This will be across a variety of venues in Rutland
Identifying early and supporting, those children, young people and families with emerging needs
Working closely with the Living Well Rutland teams, Libraries and Adult & Community Learning as well as partners including Social Care, Family Help, SEND, health and voluntary sector organisations
Contributing to performance monitoring, evaluation and continuous service improvement

Relevant experience of direct work with children, young people and families, including both 1:1 and group work
Experience of creativity and innovation.
Knowledge of safeguarding legislation, Early Help thresholds and multi-agency working
Experience of working with a wide variety of agencies in an effective way to improve outcomes for children and families.
Excellent communication and organisational skills.
A commitment to promoting equality, inclusion and positive outcomes for children and families
