Young Person and Family Outreach Worker

Are you passionate about making a difference to the lives of children, young people and families? CYPMH Dorset is seeking an enthusiastic, compassionate and resilient Young Person and Family Outreach Worker to join our developing service. This is an exciting opportunity to support children and young people experiencing mental health difficulties, helping them remain engaged with services, reduce the risk of deterioration and achieve positive outcomes in their communities. Working as part of a multidisciplinary team, you will provide community-based outreach support to children and young people with complex mental health needs. You will work closely with families, carers, education, social care and partner agencies to deliver person-centred, strengths-based interventions that promote recovery, resilience and wellbeing. This role is largely community focused, supporting young people in their homes, schools and local communities across Dorset. This role aligns with Dorset's THRIVE approach, ensuring children and young people receive the right support at the right time, with a strong focus on collaboration, shared decision-making, prevention, early intervention and enabling young people to achieve their goals. You will play a key role in supporting engagement and helping young people access the networks and resources they need to thrive. Within your Supporting Information, please indicate your preferred work base location within Dorset. This can be discussed at interview., You will:

• Support children and young people receiving CYPMH interventions to remain engaged in treatment and achieve identified goals.
• Deliver outreach support within homes, schools and community settings.
• Assist registered professionals in delivering therapeutic and behavioural interventions.
• Work collaboratively with families, carers and partner agencies including education and social care services.
• Contribute to assessments, care planning, risk management and review processes.
• Promote safeguarding and ensure the welfare of children and young people remains central to all aspects of practice.
• Maintain accurate clinical records and contribute to positive service development.
