Work and Health Employment Coach

This is an exciting opportunity to join an established team within Worcester City PCN. The role will give you a unique chance to work as part of a multidisciplinary team, whilst also building links with a range of other local services and be part of a county wide Work and Health coach network. We will particularly welcome applicants with lived experience and those with strong connections to Worcester City., The Work and Health Employment Coach supports individuals with physical, mental health, or social barriers to enter, remain in, or return to employment. Working as part of a multidisciplinary Primary Care Network team, the post holder manages a caseload of participants, conducts assessments to identify barriers to work, and develops personalised action plans to help individuals achieve their employment goals.

The role involves providing one-to-one coaching, health and employment guidance, coordinating referrals to specialist services, and working closely with healthcare professionals, employers, Jobcentre Plus, and community organisations.

The Employment Coach is also responsible for maintaining accurate records, monitoring participant progress, delivering group support sessions where appropriate, and ensuring individuals receive timely, holistic support to improve their health, wellbeing, and employment outcomes.

Worcester City Primary Care Network first became established in 2019 and consists of nine local practices working together to provide the best possible care to their patients. The PCN covers a population of approximately 131,000 patients across the city. We are committed to the development and delivery of coordinated healthcare, working with partner organisations to support patients in making informed decisions about their personal care promoting health and wellbeing and ensuring equality of access for all. We employ over sixty additional roles staff to support our general practice colleagues to deliver the right care to our patients.

It means providing innovative services that are designed around the needs of our community. We continue to work closely with our ICB and other local health and social care providers to drive our PCN towards an integrated neighbourhood team.
