Full Job Description

The MDT will consist of mental health practitioners, wellbeing workers and substance use workers. The team will provide support and interventions to prevent young people feeling overwhelmed with the day-to-day challenges of managing their wellbeing, childhood trauma, or self-medication with substances. Young people will have better access to specialist services, will feel better able to manage their emotional wellbeing, mental health and reduce any harmful substance use.

Nottinghamshire County Council (NCC) Children and Families Commissioning and the Contracts team in collaboration with Nottinghamshire Healthcare foundation trust and Change Grow Live, intend to build on the existing 16+ Supported Accommodation model by integrating a multi-disciplinary team (MDT) to deliver a specialist psychologically informed service to young people in the Supported Accommodation model.

The need to do something different arose from the strategic review of 16+ Supported Accommodation provision where it was identified that young people and commissioned providers would benefit from access to specialist mental health/wellbeing and substance use specialists. Nottinghamshire County Council made a successful bid for DfE funding to provide a Staying Close Offer in Nottinghamshire. A key component of the bid was the creation of the Multi-Disciplinary Team( MDT).

Research and evidence has shown that a high proportion of young people in supported accommodation have experienced significant childhood trauma and that past trauma affects young people in the present day. This can lead to challenging behaviour in supported accommodation services which may contribute to breakdowns in accommodation placements., The successful candidate will be required to work form Sherwood Energy Village, Ollerton on Mondays. Young people who are supported by the team will live in accommodation services, commissioned by Nottingham County council in Newark and Bassetlaw., #TeamNottsHC comprises over 11,000 dedicated colleagues who #MakeADifference every day.
