Full Job Description
We are looking for a motivated and compassionate Band 6 Specialist Wellbeing Nurse to join our team. This is an exciting opportunity to support people with severe mental illness to improve their physical health, wellbeing and overall quality of life. The role requires excellent clinical knowledge, strong communication skills and the ability to work collaboratively with GP practices, patients, carers and wider health and social care partners. You will provide specialist advice, undertake holistic assessments, support health promotion and contribute to the development of high-quality, person-centred care. This role would suit a confident and proactive nurse who is passionate about reducing health inequalities, improving access to physical health care and making a meaningful difference to peoples lives. In return, you will be part of a supportive, values-led team committed to professional development, service improvement and delivering safe, effective care within the community.Main duties of the job The successful candidate will deliver high-quality, person-centred physical health assessments and health promotion support for adults with severe mental illness, working across clinic and community settings. They will manage a delegated caseload, undertake holistic assessments, identify health needs, provide advice on lifestyle, medication side effects and physical wellbeing, and support patients to access appropriate services. The role involves working closely with GP practices, patients, carers and wider health and social care professionals to improve outcomes and reduce health inequalities. The post holder will support safe, effective care delivery, maintain accurate records, contribute to audit and service improvement, and promote clinical governance, safeguarding and risk management.
Candidates should be confident, compassionate and able to work autonomously, with strong communication, organisational and clinical decision-making skills. They will also support colleagues through advice, supervision and leadership, helping to maintain a responsive, professional and supportive service.
