Full Job Description
Role: As a member of the Linking Leeds team, you will make a real difference to the lives of others. You will be a keen advocate of the benefits of supporting people with health and social care needs to access support in their community by linking them to a network of groups and activities; enabling them to have improved health and wellbeing.
The post holder will support individuals to build independence and personal resilience, enabling self -management of their health and wellbeing. This will be achieved through the creation of Wellbeing plans which will be tailored to individual need.
A full UK driving licence with access to a vehicle and business insurance will be required
