Full Job Description

You will play a key role in supporting patients recovery and promoting independence with a Home First Approach, working closely with our community services to facilitate a safe discharge and prevent hospital readmission.

+ To ensure high standards of care are given in relation to agreed standards, policies and procedures of Torbay and South Devon NHS Foundation Trust.
+ To facilitate a learning environment to ensure all staff on the ward have the appropriate skills/knowledge and support, identified by regular staff appraisal and annual personal development plan.
+ To foster an environment that focuses on maximising patients independence where possible and utilises a wide range of meaningful activity to support this, including creating a safe space for patients to freely move around the ward.
+ To be proactive with complex discharges, ensuring they are safe and that discharge is at the earliest opportunity., You will be joining the organisation at an exciting time. We are looking for an experienced and compassionate Ward Sister/Charge Nurse to lead our friendly and supportive team in managing the 30 bedded medical/rehabilitation ward within Newton Abbot Hospital. The Ward also has close links to the Harbour Unit and had direct admission beds from the frailty service. The ward has a well-established multii-disciplinary approach to care, with extensive cross boundary working to ensure patients receive personalised and expert care.

Working for our organisation
Care to join us - Teign Ward is a 30 bedded medical/rehabilitation ward within Newton Abbot Community Hospital. The ward is also currently developing closer working with the Frailty Hub by providing direct admission beds to patients assessed as requiring admission . Medical Care on the ward is provided by a team of Drs lead by GPs who work closely with the MDT to ensure safe and timely care with the the aim of supporting patients to live as independently as possible.
