An exciting opportunity has arisen for a ward sister with a special interest in the care of the elderly. Longridge ward within Cuerden unit at Chorley & South Ribble Hospital is a 15 bedded unit that focuses on providing high quality care for patients from within the local adult population with sub-acute health care needs. We are a step up/step down facility that supports recovery and recuperation following medical and surgical illness and also provides Physical Health - progressive recovery, recuperation and manages the planning of complex discharge packages.
We are looking for an experienced, dynamic nurse to join our well established multidisciplinary team. You will need to be committed to delivering a high standard of care and support change management. The successful candidate will be an excellent communicator who will motivate, inspire, promote patient safety whilst supporting the ward team to achieve targets; this will be achieved through the provision of visible leadership, ensuring a high profile and presence on the ward.
You will be an advocate for our patients, able to identify changes in need and be instrumental in planning for their current and future care. You will provide clinical and professional leadership for a ward based team of registered nurses and healthcare assistants and deputize for the ward manager in her absence., + To provide professional leadership to Longridge Ward staff and to facilitate a culture of learning and reflective practice within the ward environment.
+ To assume continuous responsibility for appropriate deployment and supervision of staff for the effective management of patients within the 15 bedded ward.
+ To support the Ward manager and deputise in their absence, when required.
+ To provide clinical leadership in the assessment, planning, implementation and evaluation of high quality evidence based nursing care to patients and to ensure that the care needs of the patients are met.
+ To provide direct clinical care to patients with identified specialist or complex needs
+ Will provide professional and clinical leadership for the designated team on Longridge War, + Demonstrate high level clinical decision-making skills and provide a diverse range of evidence based clinical care.
+ Will provide holistic care to patients, ensuring that patient medications are administered according to LSCFT policy.
+ Will support and ensure effective relationships across the health economy with partner agencies to facilitate timely patient admission and discharge from the ward
+ To provide high quality palliative care to patients and families, as and when required
+ To assess patients needs and evaluate effectiveness
+ Be able to make autonomous decisions about the management of clinical care based on assessment and differential diagnosis
+ Attend MDT meetings with the multi-disciplinary team on the ward
+ To maintain clinical competency in a range of extended skills as required which meet the needs of the patients on the ward
+ To act as advocate to facilitate the views and choices expressed by patients, their carers/relatives
+ To work in partnership with other health care professionals, voluntary and statutory organisations to ensure that the holistic needs of the patients are met wherever possible
+ To promote health education and healthy lifestyles To carry out clinical risk assessment within sphere of competence as required
