Full Job Description

You will be responsible for the leadership and development of the ward nursing team. It will be crucial as the ward manager that you will actively seek the support and positive engagement of key stakeholders in the discharge process: including but not limited to Discharge Matron, Discharge Lounge, Clinical Site Management, Intensive Home Support, Intermediate Care Teams, Therapists, Pharmacy, Integrated Discharge Service (IDS), Ward Managers and Ward Coordinators. It is essential that you you will have the leadership and effective communication skills to ensure that suitable patients as per the criterion in the Standard Operation Procedure is followed with close working with the IDS Team and MEC Bed manager/Clinical Flow Team & Discharge Matron to ensure the best use is made of the Ward., Be a confident role model for the junior nursing team Ensure improvements in patient outcomes and the safety, treating patients with dignity and respect. Ensuring best practice in infection control. In conjunction with the Matron ensure all quality improvement work is implemented and sustained. Ensure an excellent patient journey, safe and effective discharge

  • Registered healthcare professional with current registration (NMC )
  • Education to degree level, ideally in health related subject and speciality
  • Non-Medical prescriber &/or willing to under take training
  • Leadership qualification or willing to undertake
  • management & leadership experience post registration
  • Evidence of involvement in leading change and developing programmes of care.
  • work experience in a relevant professional environment
  • Experience of Safety and Quality initiatives, Incident and Risk management.
  • Proficiency with office systems and computers. Working knowledge of Microsoft Office e.g. Word, Excel, PowerPoint and power-bi
  • Able to provide good quality and accurate reports
  • Evidence of professional knowledge supplemented by post reg. specialist managerial / leadership training and CPD
  • Demonstrate effective communication / negotiation skills is able to make operational judgements involving complex facts/situations
  • Evidence of persuasive and strong influencing skills and the ability to manage conflict
  • Evidence of effective people management and leadership skills & ability to consider the wider picture
  • Proven leadership and organisational skills and evidence of leadership development
  • Knowledge of key performance indicators for clinical governance
  • Able to work hours required by the needs of the service
  • Knowledge of budgetary control and Standing Financial Instructions
  • Demonstrate appropriate behaviours and role model Cultural change in safety, change theory and behaviours
  • Able to participate in 'on call weekday/weekend rota

  • Experience in setting establishments and using workforce toolkits
  • Knowledge of workforce planning
  • Evidence of proactive involvement in the development of nurses / midwives and nursing and midwifery practice
  • Experience in clinical audit /benchmarking
  • Quality and Safety qualification

    As a new way of working in the Medicine and Emergency Care, Care Group we have designated Ward C9 as an area to cohort medically optimised for discharge. patients waiting for: Pathway 1, Home First discharge ii) Pathway 1, Reablement, Package of Care, Package of Care restarts, Early Supported Discharge (ESD) iii) Pathway 2 patients for residential care homes from Blackburn with Darwen only. iv) Waiting for the delivery of specialist equipment v) Lancashire County Council patients with a confirmed discharge date to a residential care home This model means the ward is nurse-led, with some therapist support if required, and medical cover only an as-required in-reach support for patients who may relapse. The proposal is the patients are managed to their discharge by the nurses, therapists, and discharge support teams, depending on the patients' particular needs, with a clear focus on criteria led discharge., It is important you are made aware of the following as regards your application to East Lancashire Hospitals NHS Trust
  • Please note that the salary advertised is for full time hours. If this post is less than 37.5 hours per week, the salary will be pro-rata. For posts on Agenda for Change pay-scales, new entrants to the NHS will normally commence on the first pay point of the relevant band.
