Full Job Description
The aim of a virtual ward is to reduce length of acute hospital stay and/or avoid hospital admission; this leads to quicker recovery and reduces disruption to patients', andtheir carers' lives. This means we can proactively manage people living with frailty athome, and for those that have a sub-acute episode we can improve a patient'sexperience and outcomes. In addition, this means we have more space in our hospitalsfor acutely unwell patients and reduces the pressure on urgent care., As a Healthcare Support Worker in this team, you will contribute to the assessment and implementation of programmes of treatment care/support for individuals, families and carers under the guidance of a registered practitioner. The care/support plans may include the following:
- Promoting independence/self-management and maintaining safety
- Increased functional ability
- Improvement in health and wellbeing including mental health
- Interventions in disease/condition management
- Prevention and reducing of health inequalities
- Admission avoidance
- Early supported discharge
- Contribute to case management/ treatment care plans
- Palliative and end of life care
- Venepuncture
The post holder must hold a full, valid, and current driving licence and have access to a car, as regular travel is an essential requirement of the role
