Are you looking for a dynamic and rewarding role where you can make a real difference? An exciting opportunity has arisen to join the Home 1st Urgent Community Response (UCR) team as a Band 6 Nurse/Paramedic. The UCR service provides a rapid 2-hour response to patients experiencing acute illness or deterioration, delivering high-quality care in patients' homes or care settings. The aim is to avoid unnecessary hospital admissions, support early discharge, and improve patient outcomes through timely intervention. As a Band 6 practitioner, you will work within a multidisciplinary team, using advanced assessment, decision-making and treatment skills to deliver urgent community care. You will triage referrals, complete holistic assessments, and implement safe, effective care plans tailored to individual needs. You will work both autonomously and collaboratively with system partners including GP practices, ambulance services, and community teams. The role includes responding to urgent referrals, managing falls, supporting virtual ward patients, and contributing to seamless care pathways. The service operates 24/7, requiring participation in a rotational shift pattern including days, nights, weekends and bank holidays., The Band 6 UCR Nurse/Paramedic will:

  • Respond to urgent referrals within a 2-hour timeframe, delivering rapid assessment and treatment to patients in their own homes or care settings.
  • Undertake telephone triage, making clinical decisions on suitability for the service and escalating concerns, including identification of red flag symptoms.
  • Perform holistic assessments and physical examinations, developing and implementing evidence-based care plans for short-term acute management.
  • Deliver advanced clinical interventions, including use of PGDs and non-medical prescribing (where qualified), to support diagnosis and treatment.
  • Work within a multidisciplinary team to prevent hospital admissions and support early discharge, including care of virtual ward and Home 1st patients.
  • Respond to urgent situations such as falls, working with system partners to ensure safe management and avoid hospital conveyance where appropriate.
  • Liaise with GPs, ambulance services, and health and social care professionals to provide coordinated, patient-centred care.
  • Provide clinical advice and education to colleagues, care home staff, patients, and carers to support early recognition of deterioration and promote independence.
  • Maintain accurate documentation and contribute to audit, quality improvement, and service development.
