Urgent Care Practitioner

We are seeking an experienced and autonomous Urgent Care Practitioner to join our team. This role is ideal for an experienced clinician from an ambulance, urgent care, emergency, community or primary care background who is confident in assessing and managing undifferentiated and acute presentations within their professional scope.

A key aspect of the role is providing same-day care through home visits, practice-based assessments and remote consultations. You will independently assess patients, formulate differential diagnoses, manage clinical risk, initiate treatment, request investigations and make appropriate referral or escalation decisions. An Independent Prescribing qualification is desirable.

You will work closely with GPs and the wider multidisciplinary team to support safe, effective care and help reduce unnecessary hospital admissions wherever appropriate.

Applicants must be a registered healthcare professional (HCPC, NMC or equivalent) with substantial post-registration experience and a proven ability to work autonomously in assessing and managing acute and urgent presentations.
