Urgent Care Home Visiting

As part of a team, you will work as an autonomous practitioner, beingaccountable for independent clinical assessment,diagnosisand treatment of patients with undiagnosed and undifferentiated conditions.The role will include developing audit and research projects, providingongoing supervision and supportto medical and non-medical colleagues,andplay a pivotal role in the operational development of their specialty.The post holder canexpectedprotected time to fulfil these duties andmaintaintheir continuing professional development portfolio.

This post is within our Urgent Care Home Visiting Team which support patients that require a urgent health care need. The service is 24/7, with ACP shift patterns of 0800-2030, 7 days a week.

Successful applicants will provide autonomous care to patients a part of a multi-disciplinary team.

The current vacancy is for a full time role with shift patterns of 0800-2030, 7 days a week, including bank holidays.

Successful candidates will need todemonstratethat they are able to meet the essential criteria in the person specification and can provide evidence ofsignificant experiencein their specialist field. You willbe requiredto have and an MSc in Advanced Clinical Practice or have completed the Health Education Englandsupportedportfolioroute toaccreditation. Candidates will also hold a non-medical prescribing qualificationwhereappropriate.

We are looking for people who candemonstratethe right values and behaviours and have excellent leadership and communications skills. Portfolio evidence of core capability across all four Pillars of Advanced Practice is expected.
