Full Job Description
As a Trusted Triage Assessor, you will undertake assessments with adults to identify their needs and provide or recommend appropriate low-level equipment and daily living aids. You will work directly with residents in their homes, providing practical solutions that improve wellbeing and prevent or delay the need for more intensive services. Key responsibilities include: Assessing the needs of adults and identifying appropriate low-level daily living aids and equipment. Delivering, fitting and maintaining equipment in customers' homes. Providing instruction, guidance and practical demonstrations to ensure equipment is used safely and effectively. Carrying out risk assessments and maintaining accurate case records and documentation. Making referrals and signposting individuals to other services and agencies where appropriate. Promoting the use of assistive technology and equipment solutions that maximise independence. Ensuring equipment is collected and recycled when no longer required. Working closely with health, social care and community partners to achieve positive outcomes for residents., The role involves assessment visits, equipment delivery, manual handling, driving duties and lone working.

- Experience working with vulnerable adults.
- Experience within a repair, maintenance, installation or equipment-related environment.
- Literacy Level 2 qualification (or equivalent).
- Full UK driving licence and ability to drive a long wheel based vehicle.
- Strong communication and interpersonal skills.
- Good IT and record-keeping skills.
- Knowledge of safeguarding, customer care and safe working practices.
- Ability to manage a varied workload and work independently as well as part of a team. We Are Looking For Someone Who Is: Empathetic and customer-focused. Practical and solution-oriented.
