Trusted Assessor

As a Trusted Assessor within a hospital-based service, you will work collaboratively with Case Managers, Discharge Navigators, and multidisciplinary partners across all Discharge to Assess pathways. The role focuses on supporting and facilitating safe, timely, and effective discharges from hospital into a range of health and social care services, including both community-based and building-based provision. Working as part of an integrated team, you will triage and coordinate access to the most appropriate services and support following discharge. You will help ensure individuals achieve a seamless transition from hospital to community care., This role requires a DBS check. The level of check for this role is:Enhanced DBS check with a Adults Barred List check. Please note this role involves working in regulated activity with adults. It is an offence for people who are barred from working in regulated activity to apply for roles that require them to work unsupervised with adults.
