Full Job Description
The Virtual Hospital at West Herts comprises of collaboration between West Herts and our community partners at CLCH. The purpose of the virtual hospital is to treat acutely unwell patients in the comfort of their own place of residence, by remotely monitoring them and providing home visits where necessary.
Our vision is to support the patient's journey in a way that improves overall patient experience whilst also improving bed flow in the acute wards by preventing admission and supporting early discharge.
The role of the Trust Resident Doctor in the Virtual Hospital is to support the daily management, review and flow of patient on the Virtual Ward.
Currently, the VH offers the following pathways - Heart Function (HF), Asthma, Bronchiectasis and COPD (ABC), General Medicine, Frailty and lastly a small Post-Surgical Colorectal pathway.Not only do we provide specialist care for our patients, but we work to provide strong wrap around care to prevent readmissions.

Main duties of the job
General ward work and on-going care of in-patients- including remote ward rounds for inpatients.
Review of patients referred to the virtual ward and face to face reviews of inpatients as required, to screen for suitability for VH onboarding.
Maintenance of ward lists.
With a new hospital planned for Watford, work underway to update theatres at St Albans City Hospital and plans for a neighbourhood healthcare centre in Hemel Hempstead, this is an exciting time to join us.
