Full Job Description

This is a new exciting opportunity to work as part of the palliative care team in a fixed term role for 2 years funded by The True Colours trust. This role will enhance palliative care for our joint cardiac palliative care patient caseload. A palliative cardiac clinical nurse specialist is required to provide that essential link between services and optimise the care provided to this patient cohort, who often struggle with symptom management and require end of life care., You will be joining an innovative and progressive multi-disciplinary palliative and bereavement care team. The post will involve working across organisational boundaries to serve the needs of children with life-limiting cardiac conditions across the West Midlands offering specialist care and continued development of expert palliative and cardiac care services within BWCH. The post holder will provide essential liaison between hospital and community services for babies, children and young people with life-limiting cardiac conditions acting as a specialist in signposting and facilitating appropriate, timely and safe discharge, discharge planning and care when a child has died.
