Full Job Description
A opportunity has arisen to join a well-established team within the District Nurse Triage Team at Oxford Health NHS Foundation Trust. Using sound clinical knowledge, professional judgement and risk assessment skills, clinicians will assess referrals, clearly identify urgent and non-urgent care needs, and coordinate the most appropriate response. This may include arranging urgent, same-day or routine visits from District Nurse clinicians, signposting patients to alternative services, or providing clinical advice where appropriate. The team plays a key role in coordinating timely, responsive and patient-centred care, ensuring patients receive the right support in the right place and at the right time within the community setting. The successful candidate will:o Clinically triage referrals received by telephone and email.o Use clinical knowledge, professional judgement and risk assessment skills to identify the appropriate level of care and urgency.o Arrange and coordinate urgent, same-day and routine visits for patients in the community.o Provide clinical advice and support where a face-to-face visit is not necessary.o Work closely with District Nurse teams and wider community services to ensure safe and effective care coordination.o Contribute to the ongoing development of the District Nurse Triage Desk and the future delivery of community care.
