Full Job Description
The Senior Triage Nurse will provide triage assessment, clinical support, assistance and professional expertise to patients, carers and health and social careprofessionals accessing the Community Nursing Service.
Senior staff nurses will have a minimum of 12-months' community nursingexperience.
The senior staff nurse role supports the Triage Hub team leader with the dailyservice coordination and delivery; and is pivotal in the communication within theteam. The senior Triage nurse is also responsible for providing expert clinicalsupport and guidance to other members of the Triage Team, and those accessingthe service. You will be expected to deputise in the absence of the team leader.
The post holder will be responsible for the assessment of health needs, prevention of hospital admissions and facilitate early discharges from hospital. The current hours of work are Monday to Friday 8am to 4pm, however these will alter to a 7 day per week service, 08:00 - 20:00 as the service expands.

A vacancy has arisen within the Unplanned Care Triage Hub, which has an innovative approach to nursing care. It involves quick responses to patients in their home to assess, plan, manage and evaluate the presenting issue, prevent unnecessary hospital admissions, and help to facilitate early hospital discharges. You will support the clinical band-7 with the daily leadership and management of the team; and liaise with the wider multi-disciplinary team.
The successful candidate will have excellent clinical skills, demonstrate leadership qualities, implement high standards of patient care, and ensure a positive patient experience.
