Triage Care Worker

Play a critical role in supporting new customers integration, ensuring excellent standards of care from day one of a customer's journey with HSM. Your responsibilities will include:

• Providing shortâ€'term care and support, focusing on new customer integration.
• Building strong rapport with customers and their families from the outset.
• Identifying risks, safeguarding concerns and any required interventions.
• Communicating clearly with branch teams, field care supervisors and other professionals.
• Ensuring accurate, timely documentation that meets regulatory standards.
• Representing Home Support Matters with professionalism, empathy and expertise.
  
  Strong experience in care or support roles.
• Confidence in assessing needs and making informed decisions.
• Excellent communication and calm under pressure.
• Solid understanding of safeguarding and risk.
• A proactive, organised and professional approach.
• A Full UK Driving Licence and access to own vehicle with business insurance.
  
  07:00 - 13:30 AND/OR 16:00 - 22:00
