Transplant Specialist Nurse

The Transplant Specialist Nurse will provide expert clinical leadership and specialist nursing care for patients undergoing kidney transplantation, ensuring high-quality, patient-centred care throughout the transplant pathway. The post holder will lead the development and delivery of nurse-led services, working collaboratively with the multidisciplinary team to improve patient outcomes, promote best practice, and support service development within renal transplantation.,

• Lead and develop a nurse-led transplant service, providing specialist care and support to renal transplant patients throughout their transplant journey.
• Assess, plan, implement and evaluate complex care for patients within a variety of clinical settings, ensuring care is evidence-based and individually tailored.
• Manage and coordinate a defined caseload of transplant patients, facilitating continuity of care and effective follow-up.
• Lead the development, implementation and review of transplant care pathways, protocols, guidelines and clinical standards.
• Provide highly specialised clinical advice and support to patients, carers, nursing staff and the wider multidisciplinary team.
• Deliver nurse-led outpatient clinics in partnership with medical colleagues, contributing to the ongoing monitoring and management of transplant recipients.
• Act as an expert practitioner and professional role model, promoting clinical excellence and supporting the development of others.
• Provide specialist education, training and mentorship to healthcare professionals, students, patients and carers.
• Integrate research, audit, education and practice development into clinical service delivery to enhance patient outcomes and experience.
• Lead and participate in clinical audit, quality improvement and service evaluation activities within the transplant service.
