Transition Officer

We are seeking a Transition Officer to support learners with EHCPs, SEND and other learning needs as they transition into college. Working closely with students, families, schools and Local Authorities, you will help ensure learners receive the right support from the start of their educational journey. As Transition Officer, you will:

• Coordinate transition and admissions activities for learners with SEND.
• Complete EHCP consultations and support statutory processes.
• Conduct pre-entry assessments and gather information to inform support planning.
• Develop learner support profiles and transition documentation.
• Build positive relationships with schools, parents/carers, SENCOs and external professionals.
• Support transition events, interviews, open evenings and enrolment activities.
  
  Have a relevant qualification in SEND (Learning Needs), education, wellbeing or in a related area or willing to work towards one
• Have experience working with young people in an educational or support setting.
• Knowledge of SEND and the SEND Code of Practice.
• Strong communication, organisation and relationship-building skills.
• Understanding of safeguarding and inclusive practice.
• Level 2 English and Maths (or willingness to achieve), Applicants must be eligible to work in the UK as we do not sponsor work permits, and work permits from other organisations are unacceptable as proof of right to work in the UK.
