Transfer Of Care Hub Administrator

The successful candidate will provide essential support to a group of nurses, therapists, social workers and other professionals by communicating with a variety of services, both internally and outside agencies. They will be responsible for collecting, collating and reporting a variety of data to support the organisation.

We are looking for an individual who is an excellent communicator, is organised and confident using a variety of IT systems. The post holder will need a to Support the team with data reporting, supporting on key initiatives and projects within the team whilst supporting the development and skills of the Performance Support Assistant's.
Hours per week: 30, We welcome applications from Graduate visa holders; it is important to note that you must have a minimum of 6 months before this expires at the start of your employment for training and operational reasons.
