Full Job Description

An exciting opportunity has a risen for a motivated and dynamic Nurse to join our team as a (Trainee) Advanced Care Practitioner working as part of our Integrated Specialist Palliative Care team. Working in the inpatient, outpatient and community (Virtual Ward) setting, the successful candidate will assess patients, plan, deliver and monitor the care and management of patients with complex specialist Palliative Care needs.,

  • Assess patients and plan, implement and evaluate care using specialist clinical knowledge and skills
  • Review patients and provide care in inpatient, outpatient and Virtual Ward setting
  • Contribute to multidisciplinary approach to patient management
  • Provide education and support to patients, carers and colleagues
  • Participate in audit, research and service development within the Specialist Palliative Care service.

    The successful applicant will be expected to demonstrate advanced clinical skills, participate in service evaluation and improvement, audit and research. They will also be expected to lead on education both within the service and externally.There are 3 positions available.
