Full Job Description
An exciting opportunity has arisen for a motivated Band 4 Therapy Assistant to support Community Services, with a key focus on triage and clinical support functions. You will work closely with the multidisciplinary team to ensure efficient patient pathways and high-quality care delivery.,
- Support triage processes across community services
- Contact referrers to gather additional clinical information, clarify referrals, and support appropriate prioritisation
- Assist in the screening and processing of referrals to ensure timely allocation
- Support delivery of specific delegated clinical duties set by qualified therapists
- Assist with patient assessments, reviews, and rehabilitation plans
- Monitor patient progress and escalate concerns appropriately
- Contribute to safe and effective patient flow within community services
- Maintain clear, accurate clinical records in line with Trust policy
- Work collaboratively with therapists and the wider MDT, To work as part of the multi-disciplinary team to support flow and appropriate allocation of referrals and task.
- Information gathering phone calls to referrer under guidance of B7 triage coordinator
- Supporting with admin tasks on S1
- Preparing and sending triage letters to patient, referrer, care home etc.
- Fielding queries to CCC
- Supporting community teams with accessing patient info when out on visits
- To maintain patient records using appropriate documentation for intervention carried out.
- To promote and facilitate access to community resources.
- Identify, act and minimise risk to patients and clients.
- Exchange information with patients/clients requiring support, tact and reassurance.
- Form professional relationships with staff/patients/clients and communicate and cooperates with them in a way that respects their views, autonomy and culture.
- Constructively manage barriers to effective communication and works cooperatively with patients and team members.
- Work without direct supervision of the registered practitioner in the implementation of programmes/reviews/triage tasks appropriate to the community and evaluate the effectiveness of interventions and feedback appropriately.
- Prioritise own tasks under the appropriate delegation of the registered practitioner.
- Liaise with other providers regarding care provision.
Experience in a healthcare or community setting - Strong communication skills, including confidence in speaking with referrers and professionals
- Ability to prioritise and manage workload effectively
- Understanding of patient-centred care and rehabilitation principles
- Ability to work both independently and within a team
