Are you a motivated and compassionate Registered Nurse with a passion for delivering high-quality care in the community?

We are looking for an experienced Band 6 Team Sister / Charge Nurse to join our Community Nursing team supporting patients across the Sheppey locality. This is an exciting opportunity to take on a senior clinical role within a supportive multidisciplinary team, delivering care to patients within their own homes and community settings.

You will work closely with the Team Lead and wider community teams to support the day-to-day coordination of services while delivering and overseeing high standards of patient-centred care.

As a Band 6 Team Sister / Charge Nurse, you will:

  • Support the Team Lead with the day-to-day coordination of the community nursing service
  • Provide clinical leadership and act as a role model to the wider nursing team
  • Undertake holistic assessments and deliver evidence-based nursing care within patients homes and community settings
  • Coordinate care plans and ensure continuity of care for patients with complex health needs
  • Promote patient independence and support rehabilitation and recovery goals
  • Work closely with the multidisciplinary team including GPs, therapists and social care colleagues
  • Ensure a safe and effective service through appropriate
    clinical governance and risk management
  • Maintain accurate and contemporaneous patient records
  • Communicate effectively with patients, carers and the wider healthcare team
