Are you an experienced and inspiring Social Work leader looking for a unique opportunity to develop and lead a brand-new team that will shape the future of transitions across North Tyneside?
North Tyneside Council is establishing an exciting new Transitions Team to support young people in their transition to Adult Social Care. The team will play a pivotal role in ensuring young people who may have care and support needs under the Care Act 2014, or who have acquired eligibility under Section 117 of the Mental Health Act 1983, experience a seamless, well-planned and person-centred transition into adulthood.
We are seeking an ambitious, motivated and forward-thinking Team Manager to lead the development of this new team. This is a rare opportunity to build a team from the ground up, shape its culture and practice and deliver a service that will have a lasting impact on the lives of young people, carers and families across North Tyneside.
About the Role
As Team Manager, you will provide strategic and operational leadership for the Transitions Team, ensuring that young people receive timely, high-quality support that promotes independence, choice and positive outcomes. You will be responsible for creating a positive and ambitious team culture where innovation, collaboration and relationship-based practice thrive.
Leading a dedicated team of Social Workers and a Transition Enablement Worker, you will provide reflective supervision, professional guidance and effective performance management, empowering staff to deliver excellent social work practice while supporting their ongoing development and well-being.
The team will have a diverse range of work involving young people with a wide range of needs and experiences, including those with learning disabilities, autism, mental health needs, physical disabilities, sensory impairments or a mixture of the above. Through effective leadership and quality assurance, you will ensure assessments, support planning and interventions are strengths-based, outcome-focused and compliant with relevant legislation, policy and professional standards.
A key aspect of the role will be ensuring that transition planning begins early and is coordinated effectively across services. You will work alongside managers across Children's Social Care, Adult Social Care, Education, Health, Commissioning and Housing to develop pathways that reduce uncertainty, improve experience of transition and provide young people, carers and families with confidence about their future.
