This is an exciting opportunity to develop your management skills, drive high-quality practice, and make a real difference in supporting timely and safe discharges from hospital. If you're passionate about leadership, partnership working, and delivering excellent outcomes for people, we'd love to hear from you. What you'll get to do:

  • Lead, support and supervise a team of social care professionals.
  • Oversee safe, timely and person-centred hospital discharges.

  • Provide professional advice, guidance and decision-making support on complex cases.
  • Build strong partnerships with health colleagues and other agencies.
  • Ensure compliance with statutory responsibilities, policies and procedures.

Social Work or Occupational Therapist qualification with a current professional registration.
  • Significant experience of working within Adult Social Care and/or hospital discharge services.
  • Experience of supervising, supporting or managing staff.
  • Strong knowledge of relevant legislation, safeguarding and discharge pathways.
  • Proven ability to manage complex situations and make sound professional decisions.
  • Excellent communication, partnership working and relationship-building skills.
  • Ability to prioritise workloads, manage performance and deliver outcomes in a fast-paced environment.

Do you want to help make Gateshead a place where everyone thrives? If that sounds like you, keep reading - we might be the perfect match.
  • We're delighted that you're interested in working with us. You'd be joining us at a very exciting time. We pride ourselves on our ambitious and forward-thinking approach to taking care of Gateshead and its people, and we passionately believe that our employees reflect that spirit in truly enabling Gateshead to thrive. At Gateshead Council you'd be a highly valued member of a team that recognises and respects the skills that you bring to your role, encourages creativity and passionately advocates a great work / life balance. Our actions speak louder than words, and with flexi-time and hybrid working available across many of our roles, as well as a full suite of outstanding employee benefits, we're confident that it isn't just the salary that attracts our staff. Take the challenge. Develop your career. Be the link that turns opportunity into action. The Hospital Discharge Hub sits within the Integrated Adult Social Care, Commissioning, Communities, and Culture directorate. The service is made up of qualified Social Workers, and unqualified staff who work with system partners, individuals, and their carers to facilitate discharge where possible back to the person's usual residence. The service is a fast paced, dynamic environment, with the opportunity to work across different disciplines. You will play a key role in shaping and developing the service, and system relationships. Join us and lead the way in business collaboration and innovation.
